A candidate applying for the Alabama Athletic Trainers' Association's Scholarship has requested a letter of recommendation from you as a part of the application process. Please complete this form and submit as indicated below. This information is forwarded directly to the ALATA Scholarship Committee Chair. Please be as honest and objective as possible. The applicant must request a letter of recommendation from someone other than the supervising athletic trainer. The letter should be from a person felt to be the best judge of the applicant's qualities and abilities in the areas of academics and social involvement.
